Patient Information
Patient Checklist 
- See your physician about your condition
- Get referral (prescription) for physical therapy
- Call us at OSPTA at 540-373-3031 for an appointment
- Get directions
- Bring prescription and insurance card to appointment time
- Bring all necessary medical information (x-ray/MRI reports, medication list, etc.)
- Wear comfortable clothes for access to involved areas
- First appointment will be longer than subsequent visits
- All other questions will be answered in the clinic or on website
Insurance Information
Orthopedic and Sports Physical Therapy Associates will file for all insurance carriers, with exception of those listed below.
We are participating Providers for the following:
- Aetna (both PPO and HMO)
- One Net (formerly Alliance)
- Anthem Blue Cross/Blue Shield
Care First (if individual policy is covered through local Virginia Plan)
Cost Awareness POS
Key Care
Key Advantage
- First Health (Mailhandlers, etc.)
"First Health" will be indicated on card.
- Integrated Health Plan, Inc.
- Medicare
- Southern Health
- Tricare Standard Only (not HMO or PPO)
(Tricare Prime, Only if patient has obtained distance waiver authorization)
- United Healthcare (MDIPA, MAMSI, MAPSI, Optimum Choice, and United Healthcare plans)
- VML Compcare
- Workers Comp (Must call to determine coverage)
- Other Commercial Insurances (If out of network benefits are available)
We are not providers and cannot file claims for the following insurance companies, therefore the patient is responsible for any charges incurred:
- Cigna HMO
- Healthkeepers
- Medallion
- Nylcare
- Medicaid
- Southern Health Care Net
- United Healthcare (MAMSI, Optimum Choice, MAPSI, UHC)
- Kaiser Healthplans
In order for the patient to make the best use of his/her health insurance benefits, he/she should see a participating provider.
